You've tried the individual soundbars ingame?

When I start up and load a game, I get UI sounds (menu blips, "Turn Complete", etc.), even though all sound was previously turned off. If I go into the settings, all of the sound buttons are OFF. If I then turn them ON and then OFF again, the sound goes away. This only happens for UI sounds, and maybe some ambient sounds I'm not sure. I do not get combat sounds.

The game is clearly ignoring the setting when started up, no matter what it was.

...I always leave the background music on, low, so it's possible I just never noticed all these years - I have noticed a few times volumes not responding to adjusting on the first try - you've spotted something for sure, I think...

A quick install of a fresh GOG copy, and the answer is...
It's SMAX.

Looking in "Alpha Centauri.ini" you might see these lines (the values might be different)

Main Volume=127
Voice Volume=85
Music Volume=63
SFX Volume=95

You could try setting them all to zero, save the changes, then re-open the game.
