I have tried the ds3d=0, eax=0, and combinations.
I have the latest versions of drivers from CW.
I have reinstalled and repatched.
Any other thing to try?

Bad news, my friend - it is not the sound card (I too have a 128PCI on an PII350 ASUS 100mHz motherboard running NTW4 SP4.) It is our friends at Microsoft - who have not included the latest DirectX drivers in SP4.
They are planning an SP5 - but I doubt it will include newer DirectX. The good news is that Win2K (NT5) does have the latest DirectX included - I installed SMAC (Patch 3) at work today on NT5Beta2 - and the sound was perfect. So was the rest of the game.

I support NTW software and hardware for a living, so I get ample opportunity to test drive new stuff. If someone beats me (other than Firaxis) to a fix for the sound on NT4 I will buy them the dinner of their choice. But I am convinced beyond a reasonable doubt it is a Microsoft issue, and unless the Redmond boys surprise me with NT4 SP5, we have to wait until fall for the general release of Win2K.
