I have this setting at default (allowing restart of eliminated players), but I've never seen one actually get restarted. What's the deal here?

Probably just bad luck. I've had games where I took the last city and Mariam escaped 3 or 4 times before I finally get to lock her away.

I think I read earlier that it only restarts in the first 100 turns.

Or maybe it was the other way around -- it will restart in the first 100 even if restart is disabled.

Either way, if you think about it, once you have fifty cities cranking out bombers and satellites, little old Miriam isn't going to get very far with her single one-year-old 1-strength resurrected city...

As already noted factions won't restart except early in the game. Even then they won't restart if the game decides that there is no suitable place for them to restart -- not enough land still outside the other players borders.
